The USEF Waiver form must be completed by all show participants and trainers. To save time at the show office, we ask that you print a blank waiver below for each rider and the trainer, sign and date the form and bring to the show office when you check in. You MUST PRINT the form, we cannot electronically retrieve a completed form from Horseshowing.com.
